3/11/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Client upon arrival to the home setting. Upon arrival to the home setting, she appeared to be in a content mood evident by her facial expressions such as a smile on the face. QMHP-C discussed with Ms. Hodge recent behaviors since the last session. Ms. Hodge stated to QMHP-C that she has been not being able to control her anger as she has been throwing items in the home. 2. Behavior: Client was cooperative throughout the session as she did not need any redirection from QMHP-C for her behaviors throughout the session as QMHP-C continued to work with client on being able to implement coping skills when upset or when she cannot have her needs met. Mother reports as well as observations during the session client presenting emotions such as huffing loudly when discussing her behaviors in the second portion of the session. 1.Intervention: QMHP-C engaged Client in a therapeutic activity to assist Client on anger control and how to best manage her verbal and physical aggressive behaviors. QMHP-C role-played scenarios with Client for her to understand and practice positive anger management skills. QMHP-C processed answers and scenarios with Client. QMHP-C helped Client identify various stimuli that has triggered her anger. 2. Intervention: QMHP-C assessed Client in also identifying thoughts, feelings, and actions that has characterized her anger responses. QMHP-C engaged Client in a discussion allowing her to list ways in which anger has negatively impacted her daily activities and interactions with others. QMHP-C engaged Client in problem-solving, respectful behaviors and conflict resolutions. QMHP-C assisted Client with calming skills when feeling triggered and/or upset when she is in the home, school and community setting. 1.Response: Client struggles with understanding how her behaviors are projected toward others. Client's discussed with QMHP-C that some of her behaviorâs consist or her yelling, screaming throughout the house, talking back, being verbally disrespectful. 2. Response: She indicated to QMHP-C that she is going to do try to do what she needs to and with her having a QMHP-C she behaviors will get better.
